Rehabilitation & Nursing

Our team of licensed professionals offers home modification recommendations, treatment and instruction on home exercise, patient and caregiver education, monitoring of vital signs, assistance with community resources, and rehabilitation with an emphasis on returning to independence. We work with anyone 18 years of age and older. 

Occupational Therapy, Physical Therapy, and Speech Therapy

Excel Home Health Care's physical and occupational therapists assess and provide treatment to help patients increase mobility, strength, balance, coordination, and safety. We help those who have suffered physical and/or cognitive impairment to return to their maximal level of independence. Our experienced speech-language pathologists help patients regain speech-language skills, swallowing skills, and cognitive retraining.

is pleased to announce we are a premier participant with Medicare's Quality Assurance Organization.   The Home Health Quality Improvement (HHQI)  is a national campaign designed to reduce re-hospitalizations and medication management.   Best Practice interventions are used to provide patients with teaching tools related to their specific diagnosis.  These interventions help the patient to better manage chronic health conditions and warning signs for exacerbations.
